22 A/ ADMISSION

When the client arrives on the nursing unit, the nursing staff assumes the responsibility for his or her care. In many hospitals, the nurse is assisted by the unit secretaries or ward clerks who take care of many of the clerical duties related to admissions. Those clerical duties include preparing the client's hospital chart, recording the initial vital signs on the graphic sheet, making out nursing care cards and name tags, entering the client's name on the various nursing unit records, ordering the diet, and transcribing the medical orders. A member of the nursing staff is assigned to the tasks related to the admission of the client. These include orienting the client to the unit, taking care of the client's property, and planning and carrying out the nursing care.

The doctor attending the client and authorizing the admission to the hospital is responsible for directing the medical treatment, and this should begin as soon as possible. The doctor, or, in some hospitals, residents and interns obtain a medical history, carry out the physical examination, and write the orders for medications, treatments, tests, and procedures. Often the doctor authorizing the admission sends or phones orders to the nurse on the unit so that the medical treatment begins from the moment the client is admitted on the nursing unit. The medical treatment for each client should include an order specifying the type of diet and the level of activity allowed, as well as other medications and treatments.
